Registered Nurse Licensed (RN)

We are seeking a dedicated Registered Nurse to join our Home Health team. to assume responsibility and accountability for the application of the nursing process and the delivery of patient care. The Registered Nurse (RN) demonstrates the ability to make clinical judgments in an effective and efficient manner under the direction of the Director of Clinical Services.

Responsibilities

Utilizes the nursing process to assess, plan, implement and evaluate patient care.
Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.
Document the patient’s plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions.
Performs interventions according to identified priorities, plan of care, and the hospital policies and patient care outcome standard.
Revises the plan of care according to evaluation, changes in medical plan of care, and effective/ineffective nursing interventions.
Uses clinical judgment in evaluation activities to meet patient care needs of an assigned unit/floor including establishing priorities.
Other Registered Nurse (RN) duties as assigned.

Requirements

Current Registered Nurse (RN) License for the state in which the nurse practices.
Current Health Certificate (per facility Registered Nurse (RN) contract or state regulation).
Current PPD or Chest X-Ray.
Current BLS card.
One year prior Registered Nurse (RN) experience preferred.

Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities. POSITION QUALIFICATIONS 1. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice. 3. Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing and minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Management experience not required. Responsible for supervising hospice aides. 5. Once an offer of employment is made, it is contingent upon satisfactory references, as requested, and criminal background checks by regulation. 6.
