Responsibilities:

Provide direct care to residents, including administering medications, wound care, and personal care
Develop and implement individualized care plans in collaboration with the interdisciplinary care team
Monitor and report changes in residents' condition to the nursing team and other appropriate staff members
Collaborate with other departments, such as social services and activities, to ensure comprehensive care for residents
Participate in quality improvement initiatives and maintain accurate and up-to-date records
Demonstrate a commitment to excellence in patient care and a passion for working with seniors

Requirements:

Current LPN license in the state of Illinois
Basic Life Support (BLS) certification
Ability to work effectively as part of a team and communicate effectively with residents, families, and other staff members
Strong observational and assessment skills, with the ability to identify changes in residents' condition
Ability to lift, transfer, and move residents with assistance, as needed
Basic computer skills and familiarity with electronic medical records
