About the Role

At Avir at Clarksville, you'll be responsible for:

Providing direct patient care, including assessing patient needs, developing individualized care plans, and implementing interventions to promote optimal health and well-being
Collaborating with interdisciplinary teams to coordinate patient care and ensure seamless transitions between departments
Educating patients and families on disease prevention, treatment, and management strategies
Maintaining accurate and up-to-date patient records, adhering to HIPAA guidelines and regulatory requirements

Responsibilities

Provide compassionate and empathetic care to patients, families, and caregivers
Collaborate with physicians, therapists, and other healthcare professionals to develop and implement comprehensive care plans
Conduct thorough patient assessments to identify needs, goals, and limitations
Implement evidence-based interventions to promote optimal patient outcomes
Maintain accurate and timely documentation of patient care and treatment
