Qualifications:

Nurse Practitioner

Monday through Friday 8 to 5pm, no nights, weekends, or holidays! The Nurse Practitioner provides primary care to enrolled participants by evaluating participant physical complaints and providing appropriate treatment. The Nurse Practitioner functions beyond as an advanced practice nurse and as an integral member of the interdisciplinary team. Physical attendance at an InnovAge facility and/or an InnovAge participant home is essential to performing the job duties. Evaluates participants and documents information in participant’s chart. Performs physical examination and orders and interprets diagnostic tests. Performs blood draws, EKG’s, dressing changes and any other nursing interventions as needed. Formulates an assessment of participant’s condition and provides a treatment plan, including writing prescription(s) and providing ongoing follow-up of condition.
