Job Duties:

  • Deliver nursing care based on individualized care plans

  • Administer medications and treatments as prescribed

  • Participate in care planning and assessments

  • Respond to emergencies and ensure regulatory compliance

Qualifications:

  • Current LPN or RN license in the state of employment

  • CPR certification (AHA, Red Cross, or equivalent)

  • One year of long-term care experience preferred

  • Strong clinical, communication, and organizational skills

  • Ability to lift up to 50 lbs and work flexible hours

Perform admission assessments for new admissions into contracted facilities and document information in the EMR within specified time frame. Plan the delivery of patient care in collaboration with other disciplines to provide comfort and symptom management.
