Position Overview
We are seeking a compassionate and experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our team. The ideal candidate excels at building rapport, takes a person-centered approach to care, and communicates effectively across disciplines. You’ll work in collaboration with a multidisciplinary team to deliver comprehensive psychiatric services in post-acute and long-term care settings.

Key Responsibilities

  • Conduct follow-up psychiatric visits after initial assessments

  • Prescribe medications and review diagnostic results as needed

  • Participate in team meetings and family sessions

  • Collaborate with supervising psychiatrists and facility staff to implement treatment plans

  • Maintain timely documentation and communication with care teams

  • Provide education to facility staff regarding psychiatric care and patient needs

Qualifications

  • Active, unencumbered Illinois NP license

  • Board-certified PMHNP (PMHNP-BC)

  • Master’s degree in Nursing from an accredited program

  • Experience treating mental health conditions such as anxiety, depression, dementia, and grief in adult or geriatric populations preferred

  • Comfortable working in skilled nursing and long-term care environments

  • Strong interpersonal, organizational, and communication skills

  • Ability to collaborate with patients, families, and care teams effectively

  • Commitment to patient confidentiality and professional standards

Additional Requirements

  • Participation in regular supervision with a psychiatrist
