Job Summary and Qualifications

As a Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) at HCA Healthcare, you’ll play a vital role in delivering compassionate, personalized care to patients in the comfort of their homes. In collaboration with the Registered Nurse and care team, you’ll help manage medical needs and promote their independence – caring for patients like family, one home visit at a time.

Your responsibilities will include:

Providing nursing care in the home, including wound care, medication administration, and support during procedures
Documenting assessments, interventions, and outcomes clearly and thoroughly to support safe, continuous care
Monitoring patient status and alerting the care team of any changes to ensure timely, coordinated responses
Educating patients and families to support medication adherence, strengthen understanding of health conditions, and promote greater independence at home
Collaborating closely with RNs, providers, therapists, and care coordinators to deliver high-quality, personalized care in the home setting

What qualifications you will need:

Basic Cardiac Life Support must be obtained within 30 days of employment start date

We are seeking a dedicated Registered Nurse to join our Home Health team. to assume responsibility and accountability for the application of the nursing process and the delivery of patient care. The Registered Nurse (RN) demonstrates the ability to make clinical judgments in an effective and efficient manner under the direction of the Director of Clinical Services. Responsibilities Utilizes the nursing process to assess, plan, implement and evaluate patient care. Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition. Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses. Document the patient’s plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions. Performs interventions according to identified priorities, plan of care, and the hospital policies and patient care outcome standard.
