Certified Registered Nurse Practitioner (CRNP) – Hospice

Serenity Hospice is seeking a compassionate and experienced Certified Registered Nurse Practitioner (CRNP) to join our dedicated regional hospice team. This is a primarily inoffice position focused on meaningful,  virtual and actual facetoface clinical visits with hospice patients while collaborating closely with physicians and interdisciplinary care teams.

In this role, you will play a vital part in ensuring exceptional endoflife care by conducting required hospice facetoface evaluations and supporting highquality, compliant, and compassionate patient care across our service region.

Key Responsibilities

  • Conduct facetoface hospice patient visits to evaluate continued eligibility in accordance with CMS Hospice Eligibility Guidelines

  • Collaborate with the Hospice Medical Director and attending physicians to review patient status and adjust care plans as appropriate

  • Provide clinical leadership and guidance to the interdisciplinary hospice team

  • Support symptom management strategies and clinical decisionmaking

  • Authorize medications and treatments in accordance with state regulations and patient care needs

  • Ensure accurate, timely, and compliant clinical documentation

  • Participate in Interdisciplinary Group (IDG) meetings and care planning discussions

  • Work primarily from an assigned Serenity Hospice office, with occasional local travel to patient homes or care facilities

Qualifications

  • Current and active CRNP license in Pennsylvania

  • Hospice or palliative care experience required

  • Thorough understanding of CMS Hospice Eligibility Guidelines

  • Strong interpersonal, organizational, and documentation skills

  • Ability to work collaboratively within an interdisciplinary team

  • Willingness to travel locally within the region, as needed
