Job Summary

The Licensed Practical Nurse (LPN) participates in the implementation and evaluation of patient/resident care for stable and predictable patients/residents of all ages in the home care setting. Demonstrates knowledge of nursing principles for the patient/resident in the relevant setting and possesses the ability to perform testing to assess data reflective of the patient's/resident's status and to interpret appropriately the information needed to identify each patient's/resident's requirements relative to the age-specific needs, and to provide the care needed. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in the home care setting. Ensures the health, comfort and safety of patients/residents.

Knowledge of the principles and skills needed for nursing to provide patient/resident care and treatment. Knowledge of medications and their effects on patients/residents. Knowledge of patient/residents education principles to develop a self-care program. Knowledge of common safety hazards and precautions to establish a safe environment, specific to the home setting. Skill in administering a variety of treatments and medications as directed.

Qualifications

Graduate from an accredited practical nursing program as a Licensed Practical Nurse.

If position requires IV therapy/management skills, the following is required in North Dakota: graduation from a Board approved AASPN program where IV therapy is part of the curriculum or completion of an IV Therapy course approved by the North Dakota Board of Nursing.

One year of professional nursing experience preferred.

Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges. Obtains and subsequently maintains required department specific competencies and certifications. Depending on location, a valid drivers license may be required. Basic Life Support (BLS) for health care providers required.

Description Job Description PeaceHealth is seeking an LPN Home and Community - Hospice for a Part Time, 0.80 FTE, Day position. The salary range for this job opening at PeaceHealth is $28.73 – $43.09. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc. Hiring Bonus may be available! Under the supervision of an RN, the LPN provides direct nursing care, within the scope of their license, to patients as directed by the physicians and other healthcare providers. Cooperative decision making is expected in the execution of duties. Essential Functions Implements and evaluates patient care under the supervision of an RN. Participates in the development of a patient-specific care plan. Provides direct patient care in patient homes including administering medications and treatments as prescribed. Understands and applies knowledge in edema/lymphedema management, venipuncture, management of PICC lines, implanted ports, post-surgical drains, and pulmonary and abdominal drains. Provides focused systems and wound assessment. Reports acute medical problems and condition changes to the case manager and/or attending physician, as directed. Demonstrates proficiency in communicating effectively with individuals diagnosed with mental and behavioral disorders, and to operate efficiently in an independent home setting. Documents patient care in health record according to department standards. Delegates, prioritizes, requests and accepts guidance and assistance from others to assure delivery of care in a safe and timely manner. Ensures own professional effectiveness through continued education and professional development. Assists in the transporting of patients, specimens, supplies and equipment and in the maintenance and stocking of supplies and equipment to patient homes. May triage patient phone calls, counsel patients, providing patient education, taking patient histories, administering prescribed medications, changing dressings, cleaning wounds and monitoring patient vital signs. Teaches the patient and caregivers about disease management, in accordance with the plan of care. Performs other duties as assigned. Qualifications Education Accredited State Board of Nursing LPN Program Required Experience Minimum of 1-year clinical experience as an LPN required and Preferred: Demonstrated knowledge of and ability to apply age specific principles of growth and development and life stages to meet each patient’s needs Credentials Required: Upon Hire Licensed Practical Nurse - Oregon Required: Upon Hire Basic Life Support Required: Upon Hire Drivers License and proof of insurance Skills Basic computer skills required including familiarity with MS Office applications. (Preferred) Working Conditions Lifting Patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices (NIOSH). Fine motor skills to be able to grasp and control medical equipment and perform precise procedures. Push/Pull: Frequently up to 45 lbs. force (i.e., WOW, medical carts). Perform hands on CPR at least 20 mins using 100-125 lbs. of force (National Assoc. of EMS Physicians and AHA) Frequency should be seldom or occasionally. Ability to move around area with frequent sitting. Bending/ stooping/ squatting/ reaching/ kneeling frequently. Lifting (non-patient) up to 20 lbs. occasionally and 5 lbs. frequently. Environmental Conditions Exposure to biohazard, body fluids and airborne particles. Must be able to complete tasks in a noisy environment. Mental/Visual Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted). Ability to communicate and exchange accurate information.
