Position Summary

Under the leadership of the Ohana Medical Director and the Assistant Director of Outpatient Services, the Psychiatric Nurse Practitioner is responsible for providing care to child & adolescent patients and their families with acute and chronic psychiatric disorders. Duties include the evaluation and treatments of patients including, but not limited to, performing initial and follow up psychiatric assessments; initiating, renewing, and changing medications; ordering routine laboratory, radiological, and diagnostic tests and providing initial interpretations of results; initiating or expediting requests for consultations, special tests or studies; evaluating and initiating therapy for acute and chronic mental health problems; providing education and training (individual or groups) to the patient, family & community as necessary; and communicating clinical findings to members of the clinical team. Dependent on program need, this position may also be performing duties in the Ohana IOP/PHP program, Residential Treatment Program, Crisis Stabilization Unit and Emergency Department. Duties also include documenting all findings and results in the medical record. In addition to clinical care the Ohana nurse practitioner will be available to work with the Ohana Leadership and Management on program development. The Psychiatric Nurse Practitioner is expected to maintain a productivity expectation of at least 25 patient facing billable hours per week.

Important dimensions of this position consist of excellence in patient care through observation, assessment, nursing judgment, communication skills and interpersonal relations. Must have demonstrated critical thinking, problem solving, decision-making ability and able to work independently.

Experience

Completion of postgraduate nurse practitioner fellowship in child and adolescent psychiatry nursing strongly preferred. Minimum of two years working in child and adolescent psychiatric clinical setting required. Experience with dual diagnosis patients preferred. Must demonstrate a thorough and complete understanding of psychotropic medications, side effects, and drug/drug interactions.

Education

Master’s Degree in Nursing required- Exception Nurse Practitioners’ qualified or certified as an NP in California or any other state prior to January 1, 2008.

Licensure/Certifications

Must be currently licensed in California as an NP and be certified as a nurse practitioner by a recognized national certifying body that has established standards for nurse practitioners to practice in the State of California. State of California RN and Nurse Practitioner license with specialty certification in psychiatry. Certification from either ANCC or AANP as a nurse practitioner Furnishing license issued by the Board of Registered Nursing required. American Heart Association Healthcare Provider BLS required. Workplace Violence Prevention (AVADE) certification must be obtained within the first 6 months of hire. Will complete mandatory 5150 training by Monterey County to obtain 5150 certification and obtain 5150 Professional staff authorization yearly.

Who We Are Our program uses a holistic approach that integrates physiological, developmental, psychosocial, and behavioral factors to prevent and address feeding disorders. It is staffed by feeding psychologists, registered dietitians (RD), speech-language pathologists (SLP), occupational therapists (OT), a pediatrician and a gastroenterology nurse practitioner to provide close, coordinated care and address all aspects that impact feeding. Our largest referral stream consists of infants discharged from the hospital with feeding tubes, and we provide close, coordinated outpatient follow-up starting within one week of discharge. This allows for proactive, preventative care and oral feeding progression when developmentally and medically appropriate. Instead of using structured, protocolized, consequence-based feeding interventions, we rely heavily on principles classical conditioning to establish and protect a strong foundational relationship with each aspect of oral feeding. This approach minimizes the need for more intensive intervention and supports a natural, stepwise progression in feeding skills. It enables children to reach their full feeding potential in a manner that is sustained long-term. This proactive, preventative and holistic approach has produced exemplary outcomes and has successfully transitioned medically complex infants and young children off of their tubes to full oral feeds earlier than anticipated, even in populations known to have high rates of long-term tube dependence. This has resulted in significant expansion of our program and referral stream. Job Summary Michigan Medicine's Interdisciplinary Pediatric Feeding Program is seeking a full-time Masters-level Psychologist to join our growing team. This hybrid program (in-person and virtual) provides coordinated, interdisciplinary care to some of the most medically complex pediatric cases, including infants and children with feeding tubes and those diagnosed with Avoidant/Restrictive Food Intake Disorder (ARFID). Responsibilities* The core responsibilities of this position are twofold. The provider will staff the clinic that treats infants born prematurely and with less medical complexity who are discharged from the hospital with a feeding tube. The provider will provide care alongside a RD and an OT or SLP, to support safe and developmentally appropriate feeding progression. This person in this role will take the lead on case conceptualization, preventing oral aversion by monitoring factors that impact the infant's experience when eating including physiological comfort, hunger, stress, and psychosocial factors, and will collaborate with medical providers to identify and address physiological barriers to oral feeding progression (e.g., discomfort from reflux or constipation). Documentation is complete, accurate and timely. Signs off medical record within 48 hours of seeing patient. 15. Accurately code clinical encounters with CPT and ICD-10 codes. 16. Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patients served. Demonstrates knowledge of the principles of growth and development over the life span of the patient and possesses the ability to identify each patient’s age- specific needs. 17. Performs comprehensive psychiatric evaluations that include evaluation of mental status, current psychiatric symptoms and functioning, as well as psychiatric and other relevant health histories. 18. Prescribes psychotropic medications and short-term therapies to treat mental health conditions in accordance with standardized procedures. 19. Provides brief psychotherapeutic interventions in conjunction with evaluation and management services in accordance with standardized procedures. 20. Participates in quality improvement activities; assists in maintaining compliance with The Joint Commission, State Agencies and other internal and external regulatory standards. 21. Supports and upholds established organizational policies, procedures, and objectives related to quality improvement, safety, environmental and infection control. 22. Responds to the needs of the department by performing other duties, as necessary.
