Responsibilities

Provides comprehensive nursing care within the professional scope of practice for a variety of diagnoses for geriatric patients
Conducts initial assessment on patients, develops nursing care plans based on identified patient needs, coordinates with ancillary services to assure provision of care, documents all plans, assessments, and care provided
Provides patient and family education through hospitalization, and initiates discharge planning
Administers oxygen, IV fluids, blood transfusions, prescribed medications, and titrate medications in accordance with VA policy
Observes and assesses patients’ clinical condition, recognizes and promptly reports significant changes indicative of complications
Initiates resuscitative measures in emergency situations
Operates and monitors specialized equipment including, but not limited to: oxygen, hypothermia units, and various types of suction apparatus, infusion equipment, and patient-controlled analgesia pumps (PCA), epidural pumps, sequential compression devices and vital sign monitors
Makes nursing care assignments for various skill levels of professional and assistive personnel as assigned
Prepares patients for surgical and diagnostic procedures and assists with procedures as necessary
Provides post-procedure and post-operative recovery care
Ensures that all JHACO standards for professional nursing care are met during performance of care
Completes appropriate medical record forms in compliance with VA guidelines and policies on each patient
Verifies the content and correctness of all recorded entries in patient record; all documentation/records/reports must meet or exceed established VA standards to include, but not limited to: timeliness, legibility, accuracy, content, and signature; outpatient records shall be completed the same day the patient is seen
Demonstrates sensitivity to cultural diversity, race, gender, and other individual differences in the workforce, in the Veteran population, and in the community
Other duties as assigned

Qualifications

Holds a BSN or ADN in Nursing from an accredited program, college, or university and hold a current license to practice as a Registered Nurse in any State.
Has been practicing actively as an RN in an acute or long-term setting for at least the last one (1) year.
Is capable of providing complex and comprehensive nursing care for adult medical and surgical patients
Has and maintains current AHA Basic Life Support (BLS)
Has no health or physical disability restrictions that interfere with performance of assigned duties

Sit, stand, walk, lift, squat, bend, twist, and reach above shoulders during the work shift
Lift up to 50 lbs from floor to waist
Lift up to 20 lbs
Carry up to 40 lbs a reasonable distance
Push/pull with 30 lbs of force

Registered Nurse Care Manager - Integrated Care Management

Accountable for proactive coordination and timely transition of assigned patients to the most appropriate level of care along the continuum. Impacts key results such as achieving top decile performance in length of stay, cost efficient resource utilization, preventing readmissions and unnecessary emergency room visits. Works collaboratively with physicians, nursing, members of the multidisciplinary team (such as Home Care and PCP offices), as well as other resources internal and external to the organization. Essential Functions and Responsibilities as Assigned: Performs care coordination assessments for initial assessment of patients with 24 hrs. of admission. assessments for readmission and transition planning. Works collaboratively with the social worker and other disciplines to ensure a safe, appropriate, and timely transition to the next level of care, taking into consideration the patient’s available resources. Assesses patient/family needs to reduce barriers and formulate discharge plans (e.g., LOS barriers to D/C). Identifies unsigned level of care (LOC) orders; communicates with utilization management nurse and obtains orders from providers. Reviews current DRG/LOS identified within Cerner to assess discharge planning needs with providers and identifies which family member is the point of contact. Assesses risk of readmission for specified patient populations and initiates assigned interventions that will enhance the patient’s ability to successfully transition along the care continuum. Performs discharge planning coordination/referral by making appropriate referrals to social services, ancillary departments, outpatient case management, DME, post-acute placement, and other outside agencies per Standard Operating Procedure (SOP). Acts as a liaison by collaborating and communicating daily with the physician, patient, family, nursing, and other members of the healthcare team. Actively participates in clinical case review/rounds with the interdisciplinary team. Documents in the electronic medical record (EMR): assessment, plans, interventions, barriers, and reassessments to facilitate discharges and/or transitions, m anages anticipated discharge date and e nsures all pertinent information is transferred to post-acute agency. Identifies barriers early in the patient’s stay, formulating a plan with the patient, family, internal and external members of the healthcare team, payers, and community resources. Identifies and reports avoidable day/variances and/or service delays from established plan of care to leadership.
