Cleveland Clinic Akron General is seeking a passionate and patient-focused Advanced Practice Provider (Family NP, PA, CNS) to join the primary care team at Doylestown, located in West Norton, Ohio. This is a full-time opportunity is ideal for an APP who values relationship-based care, clinical excellence, and working within a highly collaborative and supportive environment.

In this role, the APP will provide comprehensive primary care services to patients across the lifespan, delivering high quality, evidence-based care focused on health promotion, disease prevention, and the diagnosis and management of acute and chronic conditions. Responsibilities include conducting thorough patient assessments, developing and implementing individualized treatment plans, prescribing medications, coordinating care across specialties, and educating patients to support long-term wellness and self management.

The APP will work closely with physicians, nursing staff, and interdisciplinary team members to ensure seamless continuity of care and outstanding patient experiences -fully aligned with the Cleveland Clinic's mission of putting patients first.

This position offers an excellent work-life balance with a predictable outpatient schedule and no weekend or holiday requirements.

Option to work 4-10hr shifts or 5-8hr shifts.

Responsibilities:

Conducts thorough medical histories, performs complete physical examinations (where indicated), initiates appropriate lab, radiology tests or other special tests required for evaluation of illness, and scrutinizes lab data to establish a record of the patient's current health status and to develop a working diagnosis and treatment plan. Works in collaboration with the health care team.
Performs and interprets common laboratory, radiologic, cardiographic and other routine diagnostic procedures used to identify pathophysiologic processes as credentialed and privileged by the Medical Executive Committee and the Governing Body of the applicable hospitals.

Refers patients to specialists as appropriate for consultation or for specialized health resources and treatment.

APRN

Effective 05/01/2014: MSN required upon hire or within 3 years of hire for external hires and internal hires moving from a non-APRN licensed role to an APRN licensed role.
Graduate of an accredited APRN program.
If in neonatal care service area Master’s Degree is required upon hire.

Graduate of an A.R.C. approved Physician Assistant Program.
Master’s degree in Physician Assistant Studies and/or higher degree obtained from a program recognized by their practicing states Licensing Board.

Certifications:

Job Description Nemours Children’s Health- Delaware Valley is seeking an Advanced Practice Provider in the Jefferson Einstein Philadelphia Hospital location in Philadelphia, PA for the NICU. This Advanced Practice Provider will function in both an independent and expanded collaborative role by planning and providing comprehensive, complete management of a primary caseload of critically-ill neonates/infants and their families. This position includes attendance in the delivery room for high-risk deliveries and as needed upon request. Key Responsibilities Manages and provides care for the patient requiring neonatal intensive care utilizing collaborative consultation with team members, under the direction and supervision (but not necessarily the direct attendance) of an attending neonatologist. Receives a distressed neonate in the Delivery Room and initiates appropriate intervention. Performs a complete newborn physical examination and assessment of gestational age, discriminating between normal and abnormal findings. Conducts parent interviews and takes family history. Collaborates with neonatologists concerning pertinent historical events, physical findings, diagnostic and therapeutic measures initiated on all admissions. Orders appropriate laboratory tests and procedures. Performs diagnostic and therapeutic procedures necessary and appropriate for the care of the patient. Reviews and provides general care items involved in neonatal care, such as temperature stabilization, hyperalimentation therapy, feeding management, and phototherapy. Participates in rounds in NICU.
