Job Summary The RN Case Manager I is a registered nurse responsible for coordinating the care and services for an assigned unit-based patient population. The RN Case Manager I promotes quality of care, supports effective resource utilization, and facilitates the patient's progression of care to proactively plan for the patient's safe and timely discharge to the most appropriate level of care.
