Job Description Under the general direction of the Director, Manager, and Lead/Supervisor Case Management RN, the Case Management (CM) RN is responsible for coordinating all aspects of discharge planning for admitted patients, in collaboration with the interdisciplinary care team, promoting patient and family centered care, with defined methods of screening, assessing, ongoing monitoring, and interventions that advance the progression of care. The CM RN will work to improve outcomes as measured by coordinating care to the appropriately timed discharge, reduce readmission rates, and improve patient/family satisfaction. Using the Standards for Case Management as a framework, will incorporate ethical and legal tenets into the essential job functions. Position Responsibilities Assessing new patients by gathering information, reviewing diagnoses, and analyzing medical test results for anticipation of discharge needs Complete and document an initial discharge planning assessment on all inpatients upon admission, in collaboration with the Social Worker, verifying demographics and insurance Process home health orders and case management referrals for post-acute care plans timely, including but not limited to, home health services, durable medical equipment, home infusion therapies, appointment scheduling and transfers to other facilities/level of care Communicate with care team and ensure a discharge plan is confirmed, with an expected discharge date, and monitor the ongoing progression of the plan and report any resource limitations that could impact the plan Educates providers and clinical staff on the resources available for a safe discharge, including managing provider expectations Attend and actively participate in discharge planning rounds, weekly complex case reviews, and care conferences. Coordinate with home care coordinator and social worker any discharge planning arrangement needs, transportation needs, letters of medical necessity, and regulatory requirement forms Participate in medication discussions and ensure patient can obtain prescriptions, assisting with obtaining resources, if appropriate Communicate directly with the patient and family about the discharge plan and verify understanding/agreement of the plan prior to discharge Communicate and collaborate with Utilization Management regarding patient status, changes in plan of care, and risk for denials Identifies high risk patients through the initial admission assessment and creates a collaborative plan to address their unique needs Position Requirements RN/ASN required, BSN Preferred [from an accredited school of nursing] Active Florida or multi-state RN license, required Professional certification in case management, certified managed care nurse, or area of clinical specialty, preferred Minimum of 3 years nursing experience required, related clinical experience preferred Upon hire, American Heart Association BLS required About Us Nemours Children's Health is an internationally recognized pediatric health system serving more than 1.7 million patient encounters each year. The nurse provides safe nursing care to the pediatric primary care patients with minimal guidance and bases practice on identified principles and standards of care. Synthesizes patient assessments data and recognizes complex situations prior to communicating with the care team. Documentation reflects ongoing evaluation and effectiveness of patient care. Provides care that meets professional standards of practice and meets regulatory requirements. Safely administers medications and medications as prescribed by the provider. Triages patient’s calls utilizing Barton Schmidt pediatric protocols, national teleheath standards and clinical advice from the clinical provider. Job Description Come join us - Nemours Children’s Hospital, Delaware is seeking an APRN/PA to join our Clinical Research team . This APRN/PA will function independently and collaboratively to provide specialty medical care for patients from 0 through 20 years participating in Clinical Research Trials. This position is Monday through Friday 8:30 am to 5:00pm with no nights or weekends. The incumbent will coordinate, implement and complete clinical research studies or clinical trials by assisting with IRB submissions, assisting in regulatory document preparation and collection, in-servicing research teams, recruiting study participants, obtaining parental permission/assent, educating parents/participants, scheduling and conducting study procedures/examinations, collecting and recording study data, maintaining study records, interfacing with study sponsors, participation in sponsor study monitoring visits. Excellent written, phone, and interpersonal communication skills are required. Interested applicants should possess the ability to function at a high level in an interprofessional team environment as well as an independent provider. Essential Functions Include Assessment, diagnosis, management, and follow up visits for patients enrolled in Clinical Research projects. Provide concise documentation of patient encounters including diagnoses, management strategies and follow up plan. Identifies patient and family needs for additional resources and initiates appropriate referrals. Provides patient and family teaching based on identified learning needs. Evaluates compliance with the prescribed treatment regimen through phone calls, emails, app messaging, patient/family assessments, and outpatient visits as needed. Acts as patient advocate and liaison. Acts as role model for other office members to foster professional development. Utilizes advanced knowledge and specialized skill in nursing practice in accordance with the state law, hospital bylaws, and hospital policy. Demonstrates proficiency in the care of all age groups Including neonates, Infants, toddlers, school-aged children, adolescents, and young adults. Responsible for participation in departmental initiatives and programmatic development including the identification of opportunities to improve services, offer recommendations, and implement actions as appropriate and consistent with the goals of the Nemours Foundation. Participates in the conduct of parental permission/assent process for assigned studies; schedules study visits with study participants in accordance with study protocols; develops strategies to assure compliance of study participants with protocol requirements. Performs clinical data gathering and measurements and monitoring of data integrity, including data completeness, accuracy and quality. Maintains study databases. Mentors new clinical research coordinators. Participates in training nursing and ancillary study staff for study purposes; recruits, screens and enrolls sufficient number of study participants in conformance with protocol requirements. Assists in identifying, reporting and following-up on Serious Adverse Events. Assists in the maintenance of study medication accountability records; prepares clinical specimens for shipment to central laboratories. Coordinates monitoring activities with Sponsor’s representative(s). Completes Case Report Forms; responds to sponsor queries, prepares for audits by sponsor, IRB and/or the FDA/DHHS. Requirements Must be a graduate of an accredited Pediatric Nurse Program (or) graduate of an accredited Physician Assistant Program Three years of related pediatric Advanced Practice clinical experience preferred. Current unrestricted Advanced Registered Nurse Practitioner License or Physician Assistance license to practice in the state(s) in which privileges are sought (DE and PA); Pediatric Nurse Practitioner Certification via National Certification Center; BLS and PALS Certification upon hire.
