Hospice Registered Nurse / RN Case Manager Requirements

RN license in the state you work
Two years of experience as a registered nurse, hospice experience preferred
Current driver’s license
Current CPR and BCLS certification

Job Description: The RN Care Coordination Oncology Navigator ensures timely access and seamless coordination of services throughout the continuum of care for a designated patient population which includes cancer patients receiving chemotherapy concurrently or consecutively; newly diagnosed breast or lung cancer patients; patients requiring referral to a tertiary care provider; and patients with complex care. The RN Care Coordination Oncology Navigator functions as an active health advisor, educator, and advocate throughout the process of screening, diagnosis, and treatment of cancer or other conditions. This position may be responsible for facilitation of scheduling and coordination of follow-up care if necessary. This position shares knowledge with patients and families regarding internal and external support services and resources, and tracks data on patients to ensure completion of recommended treatment plan and follow-up care. The RN Care Coordination Oncology Navigator works directly with the health care team to deliver innovative health management practices to the patients and community of Aspirus. Experience/Qualifications: Knowledge of oncology medical and nursing clinical care is normally acquired through receipt of a Bachelor of Science Degree in Nursing.
