This position reports to the Director of Nursing/Health and Wellness Director and may be required to supervise other staff positions.

Job Description

Hourly Rate $19.00

Take and record vital signs.
Measure and record height, weight, and fluid intake/output.
Recognize abnormal changes in body functioning and importance of reporting such changes to a supervisor.
Assist as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living.
Transfer, position, and turn residents.
Provide skin care.
Contribute to the resident’s assessment and the plan of care.
Provide nursing rehabilitation/restorative nursing services.
Care for residents with dementia.
Follow the schedule of resident’s needs set out by supervisor.
Provide emotional and social support to residents.
Promote and protect resident rights, assist residents to make informed decisions, treat residents with dignity and respect, protect resident’s personal belongings, report suspected abuse or neglect, avoid the need for physical restraints in accordance with the current professional standards, and support independent expression, choice, and decision-making consistent with applicable law and regulation.
Inform supervisor of any resident issues or concerns.
Respect and encourage the independence and dignity of the residents.
Respect residents’ confidentiality.
Familiar with emergency equipment and procedures.
Understand duties during a fire drill and is familiar with evacuation plan.
Attend all required training, in-service, and staff meetings.
Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices.
Maintain a positive and professional demeanor toward residents, visitors, families, and co-workers.
Adhere to all policies and procedures of Senior Lifestyle Corporation.
Perform other duties as assigned.

Qualifications

A High School Diploma or General Education Degree; or one to three months related experience and/or training; or equivalent combination of education and experience
Experience working in a skilled, sub-scute, assisted living or memory care setting.
CNA licensure if required by state.
Basic computer skills.
Ability to work as part of a team and handle multiple tasks safely and effectively.
Proficient verbal and written communication skills.
Dedicated, compassionate, and energetic.
English fluency preferred.

Answer call lights promptly. Help check in all new residents and re-admissions, as well as assist in the transfer of residents to different rooms. Put resident's clothing away properly. Report any changes in resident's condition--e.g. eating habits, behavior, temperature, etc. to the charge nurse of the unit. Turn bedridden residents every two (2) hours or sooner as indicated. Wash, clean, and dry all incontinent residents. Assist the treatment nurse as assigned. Complete assignment report to charge nurse at the end of every shift. Be responsible for well-being and nursing care of all residents assigned to his/her unit while on duty. Take vital signs and monthly weights (with assistance) and record results in EMR. Document the following in the EMR for all assigned residents: a) vital signs b) weight c) meal monitoring d) bowl and bladder e) restorative records Detect and report situations that have a high probability of causing accidents or injuries to residents and/or staff. Assure that work/assignment areas are clean and free of hazardous conditions, i.e. spills, excess carts, etc. and that equipment, tools, supplies, etc. are properly stored while working, as well as before leaving such areas for breaks, meal times, and end of the work day. Assure that established infection control and universal precaution practices are maintained when performing nursing procedures. Follow established safety precautions when performing tasks and when using equipment and supplies. Assure that equipment is cleaned and prepared for the next shift. Assure that work schedules are followed. Maintain the confidentiality of resident information and honor his/her personal and property rights. Attend nursing department, Care Plan, and other meetings as directed by the DON or ADON. Attend and participate in in-service educational classes and on-the-job training programs as scheduled or as directed. Turn in Days Off and payroll special request forms in a timely manner.
