The nurse practitioner (NP) is responsible for assuring quality care and best practice for select vascular patients in collaboration with their supervising physician. In particular the NP will ensure patients receive appropriate and timely assessments, treatments, education, and disposition to support the quality initiatives of the cardiovascular service line which reflects best practice. This includes but is not limited to the CMS, ACC, STS, and other top performer initiatives. Perioperative preparation, and postoperative clinical management is conducted in collaboration with other health care team members. The NP will also work with the hospital staff to facilitate throughput for patient flow and timely discharge. The NP shall attend professional education programs to keep current with accepted changes in practice and to satisfy continuing education requirements of the state.

Position Summary Under the general direction of the Center for Medication Management Clinic Manager, provide advanced education and care to patients receiving anticoagulant therapy, hyperlipidemia management, and diabetic education. Responsible for working with physicians within the predefined guidelines approved by the P&T Committee for management of patients referred for anticoagulation therapy, hyperlipidemia management, and diabetic education.
