Responsibilities

Provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan. Perform services in accordance with the physician’s orders and the established plan of care (POC). Ensure delivery of care is compliant with the federal and state regulations.

Qualifications

  • Must be a graduate of an approved school of nursing education.
  • Must be licensed in the state where they currently practice.
  • Must have at least one year experience as a licensed registered nurse (RN).
  • Must have basic demonstrated technology skills, including operation of a mobile device.

Education and experience, preferred

  • Medicare home health or hospice experience is preferred.

Requirements

  • Must possess a valid state driver license
  • Must maintain automobile liability insurance as required by law
  • Must maintain dependable transportation in good working condition
  • Must be able to safely drive an automobile in all types of weather conditions
  • Must possess CPR certification for the healthcare provider

Patient Care Manager | Comprehensive Rebab Unit

Position Overview: The Patient Care Manager (PCM) facilitates teamwork and unit pride by advocating for an environment that promotes high patient care standards and implements strategies to ensure patient and physician satisfaction. The Patient Care Manager is a registered professional nurse responsible for 24 hour leadership of specific nursing unit(s) within the Department of Nursing. He/She works in a cooperative and consultative relationship with the Nurse Management Team and other disciplines under the direction of the Department Director. The PCM assumes authority, accountability and leadership/management responsibility for the specific unit(s) as assigned. In addition to Patient Care Manager (PCM) duties, he/she may be assigned to work house supervisor shifts as needed. Case Manager - RN Weekend

Position Overview The Case Manager focuses on clinical quality and resource management resulting in defined clinical satisfaction and financial outcomes. They coordinate patient care to a population of different ages within the Singing River Health System. Expectation is for all performed duties to be in accordance with Singing River Health System procedures and policies, accreditation organization, and governing guidance and publications for health care employees. Case Manager must be able to work effectively under pressure, multitask and be self-directed. Case Managers must have strong written and oral communication skills, analytical and critical thinking skills, conflict management and team-building skills, and strong organizational skills. The Case Manager must have the ability to perform numerous tasks simultaneously with minimal errors. Case Managers will participate in the daily discharge planning meetings: addressing barriers that may inhibit discharge; targeting successful transition from one level of care to the next. Case Managers will work to create a seamless line of communication between the inpatient care management team and outpatient case management team.
