Responsibilities

Certified Nursing Assistant CNA

As a CNA you will be responsible for assisting licensed care staff in performing a variety of clinical and technical tasks to assigned residents to include assisting with activities of daily living, light housekeeping, dining / tray services, escort services, performing vital signs and weights and other care related duties as assigned. As part of the care team, you will participate in quality assessment and performance improvement activities and notification of resident changes of condition as they occur.


Qualifications

  • Qualified applicants are required to possess current state certification and experience is required.
  • Prior experience working with geriatric clients is preferred.
  • High School diploma or GED equivalent is preferred.
  • Current CPR/Automated External Defibrillator (AED) certifications are required.
  • Current First Aid certification is required or must be willing and able to become First Aid certified.
  • Food handlers certification and Certified Medical Technician/Certified Medication Aide certification, where required.
  • Basic computer skills or ability to learn.

About the Job: Performs delegated nursing, homemaking, and other delegated duties for patient/clients of all ages and families in their home or other designated facility. May perform PT/INR checks in the home as ordered by the Physician and reports results according to policy. What You’ll Do: Reports and documents patient/client’s needs and responses to the appropriate Case Manager and/or Clinical Manager. Reports patient/client’s change in condition or situations immediately. Adheres to patient/client Home Health Aide care plan as set up the primary nurse/therapist while maintaining a healthy, safe environment. Reinforces the teaching of the primary nurse/therapist by reviewing personal care techniques with caregivers and patients. Completes documentation in clinical records, time sheets and quality control logs. Attends a minimum of 12 hours of in-services per year. Provides care while promoting client/patient dignity and provides a calm, supportive and therapeutic environment for client/patients and their families. Who You Are: Education: High school diploma or equivalent preferred. Experience: Six (6) months to one (1) year of experience in a related field desired. Certification/Licensure: Current Nursing Assistant License in the State of New Hampshire required. Schedule: Work schedule may include 8 to 12 hours per day with weekend, evening, night, and on-call coverage based on VNA needs. Staff are expected to cover for absences by rotating or working extra time. Staff are assigned to the service area of the VNA to meet patient/client needs. Other: Driver's License required to travel throughout New Hampshire to provide home health care service and other job-related activities.
