A clinically competent, registered, professional nurse who delivers care to patients through the nursing process of assessment, planning, intervention, implementation, and evaluation. Effectively interacts with patients, significant others, and all other health care team members, while maintaining standards of professional nursing. (Washington State Nurse Practice Act, WAC, ANA Code of Ethics).

 

Position Reports To: Unit Clinical Manager/Director


Essential Functions

  1. Perform related assignments as requested in order to promote positive visitor, family, and co-worker relations.
  2. Coordinates and provides high quality patient care in a caring and compassionate manor, utilizing the nursing process and unit specific standards of practice
    • Accurately collects patient information needed to provide safe, high quality care. Completes holistic ongoing assessments of patient’s status and responds appropriately.
    • Develops, implements and continually reassesses patient’s individualized plan of care.
    • Provides nursing care for patients including administration of medications, completion of nursing procedures and/or treatments.
    • Determines and completes appropriate nursing actions to meet care goals; including the physical, psychological, social and rehabilitative needs of the patient.
    • Teaches patient/family and/or significant others appropriate health information in a timely manner and shares written informational material.
    • Continual evaluation of discharge plan for appropriateness, educational needs, and changes as needed.
    • Documents care provided, assessments, plan of care, interventions and responses accurately in the system.
    • Demonstrates ability to handle emergency situations in a prompt, precise and professional manner.
  3. Actively maintains current knowledge base of workflows, process and evidence-based procedures
  4. Delegates appropriately and oversees care provided by non-licensed assistive personnel.
  5. Participates in performance evaluation/improvement processes with Director/Clinical Manager. Assumes responsibility for own professional development and personal growth.
  6. Consistently functions within the guidelines of the Washington Nurse Practice Act, unit specific standards of practice, and keeps current with the legal aspects of nursing.
  7. Communicates and collaborates with members of the healthcare team including physicians, patients and families to facilitate effective patient care and treatment.
  8. Maintains a clean and safe environment for patient and staff according to hospital and unit policies
  9. Actively participates in quality improvement programs.
  10. Adheres to Confluence Health and Wenatchee Valley Hospital or Central Washington Hospital policies and unit standards as applicable. Required to maintain active Basic Life Support (CPR) certification.
  11. Required to maintain active Basic Life Support (CPR) certification
  12. Other duties as assigned.

Qualifications

Required:

  • Graduate of an accredited school of nursing (NLN standard).
  • Current licensure in the state of Washington (RCW 18.88).
  • Must possess basic computer skills related to Windows navigation, mouse usage, keyboarding, email communication and password management.

Summary The Nurse Navigator is accountable for navigation and coordination of care utilizing evidenced-based clinical pathways for cardiology patients along the continuum of care for their cardiac condition from diagnosis through treatment. They will provide longitudinal comprehensive nursing care to a designated case of cardiac patients. They will have primary nursing responsibilities for the coordination of care, symptom management and education of patients and families. This position will work in collaboration with all members of the healthcare team to achieve the best possible outcomes for the patient. Position Reports To : Practice Manager Essential Functions Manage, develop, implement and evaluate initial patient plan of care following clinical pathways in collaboration with the physician and the health care team, to determine the effective utilization of resources and outcome achievement. Facilitate and/or conduct patient, family, and team conferences to develop and monitor a plan of action as necessary. This includes patient education on the disease, treatment options, procedures and medication regimens and side effects. Ongoing assessment of patients for unmet social needs with appropriate referral and communication to social services, community resources, etc. Determine need for further diagnostic work-up; communicate and coordinate necessary tests or procedures to the patient and the healthcare team. Obtain, review and scan outside patient records for new newly referred cardiology patients. Coordinate simple and complex patient care issues in conjunction with other parts of the healthcare system, working interdepartmentally as well as with other members of the cardiovascular team. Determine timeliness/time appropriateness of appointments. Ensures effective and appropriate utilization of resources and efficient patient throughput along continuum of care. Coordinate and continue to develop an established virtual care/telemedicine program for patients in the region, in conjunction with community clinical resources. Assist with patient registry work as it pertains to chronic disease management. Document interim progress notes reflecting coordination of care/services. Intervene when the clinical pathway is not being followed, either due to patient constraints or ineffective follow-through. Provide triage services and symptom management to patients as needed, in-person, telemedicine or over the phone.Communicate symptomology to appropriate team members. Required to maintain active Basic Life Support (CPR) certification. Other duties as assigned. Qualifications Required: Current licensure in the state of Washington (RCW 18.88) or licensure through Multistate Nurse Licensure Compact (SSB 5499). Current BLS Health Provider and ACLS card. Competent in basic computer skills. 3 years of clinical experience. Ability to work independently and perform role with minimal guidance Desired: Previous department experience.
