As the Director, Nursing, the work you do every day makes a difference in the lives of our patients by assuming ultimate accountability and leadership for the assessment of the patient condition and plan of care. The Director of Nursing provides oversight for case management, clinical care and is responsible for coordination all clinical disciplines involved in providing quality, cost-effective and billable care through direct care and supervision of care under the direction of the physician. The DON provides direct supervision to field staff and internal clinical support functions to achieve clinical excellence outcomes, regulatory compliance, patient, and employee satisfaction, in a fiscally responsible manner.

HOW YOU'LL WORK:

You’ll provide oversight of the coordination of all clinical disciplines involved in providing quality, cost-effective billable care under the direction of the physician. You will be responsible for maintaining relationships with our partners to achieve clinical excellence outcomes, regulatory compliance, patient, and employee satisfaction, in a fiscally responsible manner.


MAJOR AREAS OF RESPONSIBILITY:
Leadership/Development: Directly supervise, lead, and develop the Clinical Managers, and indirectly guide the field staff and clinical support team.
Outcomes: Monitor patient trends and outcomes for the agency.
Communication: Interact with individuals (internal clinical teams, patient family member, physician, etc.) as applicable via phone, email and/or face to face concerning patient care.
Customer Service: Work with Clinical leaders to ensure patient and employee satisfaction and resolution of issues and concerns.
Policies: Assume accountability for policies/procedures, documentation, and clinical charting practices in accordance with agency protocol and Medicare/Federal/State guidelines.
Fiscal Responsibility: Deliver results for the budget and other key performance indicators such as admissions, recertifications, HHRG, etc.
Collaboration: Collaborate with our health partners and Care Integration team with quality and process measures relating to star ratings, hospital utilization and patient satisfaction.

REQUIREMENTS:
Registered Nurse in the state of employment
A minimum of three years of supervisory or administrative experience in home healthcare, or related health programs
Expertise and Homecare background for multiple sites preferred
Knowledge of guideline governing home health agencies
Knowledgeable on nursing best practices
Operational knowledge of HCHB software, PDA’s, ADP, Microsoft Office & Outlook preferred
Valid driver’s license and auto insurance
Capable of all physical demands

Trauma Performance Improvement Specialist Registered Nurse

Position Summary: The trauma PI specialist supports the Trauma program's Performance Improvement and Patient Safety (PIPS) activities, including reviewing of trauma care across the continuum of care, case finding, and audit of medical records. Provides coordination and facilitation of performance improvement activities, including trending and benchmarking of data outcomes. Essential Functions and Responsibilities : Under the general supervision of the Trauma Program Manager and Trauma Medical Director, functions to included, but not limited to: Performs and/or supervises medical record audits related to various trauma quality improvement indicators to identify problems and/or trends in trauma care for referral to providers including but not limited to data for the MTQIP (Michigan Trauma Quality Improvement Program) and TQIP (Trauma Quality Improvement Program. Responsible for providing feedback to referral hospitals/physicians regarding patient outcomes. Will attend 2 of 3 tri annual MTQIP collaborative meetings yearly. Regularly attends Trauma Multidisciplinary Peer Review Physician meeting Prepares chart reviews and summaries for Trauma Peer Review/ morbidity and Mortality. Regularly attends the Trauma Multidisciplinary System meetings. Develops and assists others in developing action plans and ensures event resolution. Assist the Trauma Program Manager and Trauma Registrar. Ensures the accuracy and completeness of trauma registry, including data abstraction, entry, and submission to the database. Reviews trauma patients clinical records including EMS reports, ED, surgery, and inpatient records. Assist to design and provide structured audits and reports of the registries data validation. Make recommendations to improve the process of data collection for the registry.
