Job Description

The Nurse Practitioner Clinic examines and treats patients independently and in autonomous collaboration with other health care professionals following established standards and practices. Ensures proper illness and injury care and disease prevention, diagnosis, treatment, and recovery. Relies on education, experience, professional training and judgement to accomplish responsibilities. A wide degree of creativity and latitude is expected. Works under minimal supervision under the direction of the collaborating physician, as appropriate. May lead and direct the work of others within scope of practice.

Responsibilities

  • TECHNICAL TASKS
  • Demonstrates competence in all aspects of practice within the scope of a family or adult nurse practitioner. Uses appropriate internal and external resources to resolve, meet or exceed patient and family health care needs
  • Demonstrates competency and accuracy in all work tasks, including those directly and indirectly related to patient care, consistently providing nursing care according to established practice guidelines and in conformance to established standards of care and administrative policies & procedures
  • Identifies common and unusual problems for patient populations in order to develop expected outcomes
  • Maintains current knowledge concerning principles and practice of nursing through current literature, attendance at workshops and seminars, membership in professional organizations
  • Creates and maintains accurate records, appropriate legal documents and other reports of client care consistent with the requirements of local, state and/or federal regulatory agencies
  • Is responsible for adhering to the Standards of Nursing Practice and developing, or assisting in the development of Quality Assurance indicators for the practice
  • At all times, maintains complete and accurate recordkeeping of all patient care related activities, assisting as necessary in the patient billing process, providing information relative to office visits to appropriate personnel within the office
  • Demonstrates proficiency in the referral process and use of multidisciplinary resources to meet patient needs; maintains, at all times, appropriate channels of communication and harmonious relations with other healthcare providers.
  • Demonstrates competence in operation of routine office practice equipment and consistently operates them in a safe fashion
  • Completes requests for office visits, outside patient testing and routine information in an accurate & timely fashion
  • Demonstrates a knowledge of corporate policies regarding fire and safety, handling hazardous materials (if applicable) and infection control.
  • Participates and contributes to the Quality Assurance and Infection Control Program, as appropriate.

  • Able to assess, diagnose, develop plan of care, and assess patient outcomes in a competent and timely manner
  • As appropriate, requests and assists the collaborative physician in development of a patients plan of care
  • Performs reassessment of patients, patient progress, implications for revision of plan of care, and documents same
  • Assesses & facilitates resolutions of the patients psychosocial problems and support needs, initiates necessary consultation and involvement of other health care personnel, as appropriate
  • Discriminates between normal and abnormal findings on history and physical examinations, recording these findings in the chart/record
  • Effectively plans the use of time at work, setting priorities, assisting others as necessary, and is creative in the use of extra time
  • Performs necessary diagnostic and therapeutic or emergency measures as indicated for identification and management of problems, including cardiac arrest, respiratory arrest/distress, injuries, burns, hemorrhage, removal of superficial foreign bodies, application of dressings and bandages, and basic hearing & vision testing.
  • Institutes appropriate emergency measures for the stabilization of patients with apnea, shock, and respiratory/cardiac arrest.
  • Inspires confidence from patients, visitors and families by performing and communicating in a courteous and professional manner at all times.
  • Displays good problem-solving skills and takes responsibility for improving technical skills, critical thinking and interpersonal relationship skills.
  • Communicates impressions, plan of medical management and information about patients with the collaborative physician as appropriate
  • Plans, implements and evaluates all nursing care consistent with the scope of practice of a nurse practitioner and in conjunction with the collaborative practice guidelines
  • Utilizes initiative and independent judgement in evaluating the assessment, implementation and evaluation of patients coming into contact with the practice.
  • Plans individualized patient care for patients that reflects continuity of care and family involvement; identifies nursing interventions, develops creative options to meet needs of clinic patients and their families.
  • Develops Standards of Nursing Care and Standards of Nursing Practice and Quality Assurance criteria to assist with evaluation of patient care.
  • Collaborates with appropriate personnel in developing, implementing and evaluating a comprehensive plan for patients and, as appropriate, their families
  • Utilizes interviewing skills as a tool for obtaining data from specific patients with complex problems, then formulating a plan for resolution.
  • Participates in the medical care of patients seen in the office, assessing and developing a comprehensive health data base (including the elicitation of a comprehensive health history and performance of physical assessment, using skills of observation, inspection, palpation, percussion and auscultation, as well as basic instruments and indicated screening procedures).
  • Sets priorities, with appropriate personnel, for matters related to patient care
  • Assesses the health care delivery system in the office, in relation to its appropriateness, with a view to changes and revisions of policies and procedures, if warranted
  • Initiates or modifies medical treatment.
  • Helps assess the medical and nursing care rendered by self & others within the practice, identifying any need for improvement and communicates this information to appropriate personnel.
  • Accurately assesses health status of patients coming to or telephoning the practice for assistance and/or information.
  • In collaboration with appropriate personnel, assists in the development and implementation of teaching plans for patients & their families.
  • Other duties as assigned

Qualifications

Education - A graduate of an approved professional school of nursing. Completion of State Board of Nursing approved Nurse Practitioner Course and license or temporary permit to practice professional nursing as an Advanced Practice Registered Nurse.

Central Intake Clinician Days PRN

The Central Intake Clinician responds to inquiry call and/or emails/faxes. Conducts initial intake assessments, provides community referrals when appropriate, and coordinate admissions processes. Handles all aspects of managing the patient flow from the initial inquiry, assessment, documentation, internal and external communication and coordinates admission or referral to the appropriate level of care based on the patient’s clinical presentation. Complies with Joint Commission, Federal and State Regulations, Oceans' Mission, policies and procedures and Performance Improvement Standards. Essential Functions: Responds to inquiry calls, completes inquiry forms accurately, completes assessments in a timely manner and documents follow up calls made regarding pending assessments. Makes every effort to honors all patient facility preference requests. Inputs all initial referral requests into EMR system to initiate insurance verification process to be completed on all perspective inquiries. Conducts phone assessments, completes Intake Screening and Assessment Part 1 thoroughly on all appropriate accepted patients. Uploads all pertinent referral documentation into document storage in EMR system. Refers patients who do not meet criteria for facility programs to appropriate community resources. Refers all patients that meet outpatient criteria to the nearest outpatient program available to them. Communicates with patients, families, direct referral sources, and internal Oceans personnel in professional manner sensitive to their age, disability and cultural needs. Delivers patient services in a respectful manner which promotes dignity and patient satisfaction. Identifies safety needs including ability to detect possible signs of abuse and neglect and report same to Supervisor on duty. Contacts emergency services on behalf of patients in crisis when necessary for wellness check, while keeping the patient engaged on the phone until emergency services have arrived, and patient is safe. Identifies special mobility and equipment needs of patients. Identifies need for interpreter services, when necessary, prior to admission and notifies accepting facilities of need. Assess and interprets age and disability needs to assess level of care needed and place patient at nearest appropriate facility that can accommodate the patient's needs. Communicates with providers in a professional manner to include all pertinent information regarding the patient's presentation for provider to determine acceptance or denial based on criteria provided. Communicates with patients and families in a manner sensitive to their age, disability and cultural needs and involves families of minors or significant others in decision making related to treatment as appropriate. Maintains real time progress of referrals and updates facilities, as necessary. Requests assistance from the facility Clinical Liaisons when necessary to obtain further collateral information from referral source. Performs other duties and projects as assigned.
