Job Description
The Nurse Practitioner examines and treats patients independently and in autonomous collaboration with other health care professionals following established standards and practices. Ensures proper illness and injury care and disease prevention, diagnosis, treatment, and recovery. Relies on education, experience, professional training and judgement to accomplish responsibilities. A wide degree of creativity and latitude is expected. Works under minimal supervision under the direction of the collaborating physician, as appropriate. May lead and direct the work of others within scope of practice.

Responsibilities
- TECHNICAL TASKS
- Perform bedside palliative care patient/family evaluation.
- Assist with diagnosis, treatment, and symptom management of acute and chronic health conditions.
- Order, perform, and interpret treatments within scope of professional practice.
- Consult program medical director as needed; comply with applicable laws and regulations with respect to Collaborative agreements.
- Initiate reimbursement for services rendered
- Assist in development of clinical practice guidelines/standards/protocols in support of quality palliative care.
- Provide training and continuing education for staff on palliative care.
- Meet monthly to provide reports and outcomes in relation to Palliative Care team goals and objectives.
- The leadership role of the Practitioner includes patient and family expectations; group dynamics to affect positive change; and provides a strong image as a role model. The practitioner will also supervise all RNs who are assisting with the program.
- INTERPERSONAL RELATIONS
- Communicates and collaborates with physicians, nurses and other multidisciplinary team members involved in the care of patients.
- Acts as patient advocate constantly monitoring, tracking, assessing and evaluating quality of care
- Promotes communication with referring physicians and the entire team regarding plan of care
- Promotes professional development of the team.
- Other duties as assigned
- Consistently performs 12 organizational Service Standards focused on Values, Service, and Quality.
- Program development experience
- Current license to practice profession nursing in Louisiana Master's degree in Nursing and/or Nurse Practitioner license
- Current license to practice as APN in Louisiana. Current BLS Certficiation
