Schedule & Setting
Full-time position with a structured weekday schedule. You'll spend 3 days delivering on-site care across Stockton, Modesto, and Amador, with strategically clustered routes designed to minimize travel and maximize time with patients. The remaining 2 days are remote, giving you the flexibility to work from home for administrative and documentation tasks.

Qualifications

Compensation

Annual compensation range: $150,000–$175,000 (based on experience, schedule, and patient volume)
