As a Nurse Practitioner (NP) or Physician Assistant (PA) at CenterWell Senior Primary Care, you will be part of a collaborative, multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care.

You will work onsite at our South Zarzamora clinic in San Antonio. You will report to our Associate Medical Director for South San Antonio.

What We're Looking ForRequired:
- Master's Degree in Nursing or completion of a PA program with board certification.

Preferred:

- Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers
- Familiarity with HEDIS quality indicators and Medicare guidelines.

You will work in the Corpus Christi area and report to the National Medical Director for In Home Care Purpose of the Role: As an Advanced Practice Provider (APP) within our Integrated Home Care Program, you will play a pivotal role in enhancing the well-being of frail, medically complex, and often homebound patients.
