This job is an integral part of the care delivery system working together to provide a healing environment, by placing our customers first. The incumbent provides primary healthcare services for non-acute or chronic health conditions in specific populations in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization within the CRNP scope of practice. Work is performed in a physician office or offices dedicated to Primary Care, weekend, and on-call coverage on a rotational basis, as required.

ESSENTIAL RESPONSIBILITIES

Provides appropriate patient care to include recording patient medical history and physical assessment, developing therapeutic care plan, ordering diagnostic tests, interpreting test results, providing accurate medical record documentation. 25%
Communicates and coordinates patient care with patient, family, community health services and interdisciplinary team. Involves attending physician regarding plan of care and involves physician in clinical decision making of any issues that are outside the CRNP's knowledge base or scope of practice. 15%
Provides monitoring and continuity of care and coordinates periodic evaluation by physicians as defined in the written agreement between the CRNP and the supervising physician(s).10%
Prepares written prescription orders for drugs and controlled substances, administers medication and injections, sutures wounds and provides wound care10%
Educates patients and family members regarding health and preventive medicine, medications prescribed, treatment instructions and recommends community resources as appropriate.10%
Triages patient telephone calls, provides monitoring and continuity of care10%
Participates, develops, and implements performance and/or quality improvement initiatives. Ensures compliance with all regulatory agencies.10%

Provide administrative duties to include but not limited to opening/closing the clinic, inventory, patient follow up, verifying insurance and collecting payment, maintaining a clean environment. Be responsible for handling patient appointments and performing routing examinations to ensure positive patient health outcomes Perform duties including speaking with patients about healthy lifestyle choices, completing diagnostic tests to help diagnose and treat patients for injuries or illnesses and coordinating with other healthcare professionals to maintain a comprehensive plan of care Encourage patients to become active participants in their own healthcare and well-being through services such as health screenings, behavioral health and wellness, chronic condition management Required Qualifications A minimum of one year experience as a Nurse Practitioner is required. Experience in Family medicine or primary care practice preferred. Active Family Nurse practitioner license in good standing Provide holistic, evidenced-based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients. This includes education and treatment for pregnancy prevention, sexually transmitted infection (STI) Prevention and safer sex practices. Order and administer immunizations and injections (to include but not limited to COVID19, influenza, TB, Depo) and provides evidence-based patient/parent vaccine counseling and education. Minimum of two years of medically relevant experience or equivalent Complete the DOT certification exam within 30 days of employment Depending on the market, the ability to be proficient in both speaking and writing in additional languages not limited to but including Spanish Basic Life Support (BLS) certification, including but not limited to performing CPR and operating an Automated External Defibrillator (AED). Pass a FIT test and wear the provided respirator mask when protocol requires. Preferred Qualifications EPIC experience preferred Education Master's Degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse role Pay Range The typical pay range for this role is: $41.85 - $90.13 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
