Job Description Summary

The Postural Orthostatic Tachycardia Syndrome (POTS) Clinic RN Coordinator is responsible for performing individualized and age-specific nursing assessment of assigned patients using advanced assessment skills and other medical data to formulate nursing diagnoses. Develops individualized care plans and provides and coordinates comprehensive nursing care. Instructs patients and significant others in care provided, disease and/or illness, expected outcomes and home care.

The POTS Clinic RN Coordinator supports patients referred for evaluation of postural orthostatic tachycardia syndrome and related autonomic conditions, assisting with intake, visit preparation, symptom management, education, and coordination of care during the diagnostic process. Following physician evaluation, the RN plays a key role in care transitions, supporting patients and families with clear care plans, education, and follow up recommendations and communication.

Job Description

The POTS Clinic RN Coordinator will:

  • Provide professional nursing care for the comfort and well-being of patients, which includes performing specialized treatments, administering prescribed medications, and coordinating patient care with other departments.

  • Collect pertinent physiological, psychosocial, and functional data to formulate nursing diagnosis using evidence-based techniques and instruments. 

  • Assist physicians during evaluations, testing and treatments.

  • Formulate nursing diagnoses by interpreting data from health team members and integrate into written assessments.

  • Assess patients, identify changes in status, establish priorities for patient care, and identify crisis situations when needed.

  • Implement interventions consistent with the established plan of care and interdisciplinary treatment plan, evaluate progress toward goal attainment, and modify the plan of care as indicated.

  • Provide information to patient and significant others regarding care during hospitalization, expected outcomes, home care instructions, and referrals to appropriate health agencies and resources.

  • Prepare and provide reports on patient care plans to health team members, which includes completing assignment sheets, providing explanation of patient care plans, and completing documentation.

  • Collect and report pertinent physiological, psychological, and functional data based on patient plan of care or patient need.

  • Review, evaluate and make recommendations regarding SRAlab policies and procedures related to patient care to ensure comprehensive patient care.

  • Evaluate patient’s current pain levels (0-10) and history of recent pain experience; provide appropriate pain education and intervention in consultation with physician and pain specialists as needed; provide ongoing assessment of pain and implementation of tools for management.

  • Perform all other duties that may be assigned in the best interest of Shirley Ryan AbilityLab.

Outpatient Specific Principal Responsibilities

  • Manage prescription refill requests, obtain prior authorizations from insurance providers, and coordinate follow‑up appointments to support continuity of care and timely patient access to treatment.

  • Perform outpatient wound care, including assessment, cleaning, and dressing changes, for patients with acute injuries, chronic wounds, or post‑surgical needs, following provider orders.

  • May assist with patient preparation and procedural support for various outpatient procedures such as botulinum toxin injections intrathecal baclofen pump refills PRP procedures, MSK injections, including equipment setup, patient education, and post‑procedure monitoring 

  • Insert peripheral intravenous lines (PIVs) and administer prescribed intravenous fluids (IVF) safely and efficiently in the outpatient setting, adhering to institutional policies and standards of care.

  • Draw patient laboratory specimens during clinic visits, ensuring proper patient identification, specimen handling, and timely transport in accordance with laboratory requirements.

  • Provide timely, professional communication with patients via the electronic patient portal and telephone, addressing questions, concerns, and care needs while ensuring accurate documentation in the electronic medical record.

  • Review and triage patient messages and clinical concerns, utilizing nursing judgment to escalate appropriately to physicians or advanced practice providers to support safe, efficient clinical decision‑making.

POTS Program Research Specific Principal Responsibilities

  • Maintain Citi training requirements to fully participate in research within the POTS program.

  • Where required, collect study data and/or provide study intervention as required by study proposal.

  • Provide clinical care to participants as assigned or consulted, within the scope of the registered nurse. Such activities may include, but are not limited to, conducting medical and social histories, performing clinical assessments, collecting and processing biological specimens, providing patient education, and administering medications, investigational agents, or treatments as ordered by the supervising provider.

  • Maintain clinical database including obtaining subject consent, extracting data from medical records and querying database.

  • Review study documentation for completeness, accuracy, consistency, and compliance. Ensure accurate and timely communication of clinical data to the study team and/or sponsors as appropriate. Manage requests for data clarification or queries.

Reporting Relationships

Reports directly to the Nurse Manager or Director of Physician Practice Clinics

Knowledge, Skills & Abilities Required

  • Professional knowledge of practice acquired by obtaining a Bachelor/Master of Science in Nursing from an accredited program

  • Minimum 2 years of nursing experience.

  • Illinois license to practice as a registered nurse.

  • CPR certification.

  • CITI Training Required or ability to obtain within Probation Period

  • Understanding of the growth and development of infant, child, adolescent, young adult, middle-aged adult and geriatric patients. 

  • Demonstrate the ability to interpret growth and development related information to assure patient needs are met.

  • POTS experience preferred

  • Experience in conducting research studies preferred

  • Ability to communicate effectively using timely verbal, non-verbal and written communication.

  • Ability to take ownership for serving the customer, team and organization, for achieving goals and for resolving problems. 

  • Willingness to adapt to changing work demands and work environment.

  • Demonstrated critical thinking skills and ability to make sound judgments to monitor and respond to patient needs or changes in condition.

  • Ability to anticipate, assess and address the needs of patients

  • Ability to transfer and position patients, consistent with lifting up to 35 lbs.

  • Normal patient care environment with little or no exposure to dust or extreme temperature.

Qualifications: Demonstrates eligibility for employment in the U.S. Graduate of a school of professional nursing with current and valid licensure with Texas State Board of Nursing as a Registered Nurse. BLS & ACLS required or obtained within 30 days of hire and PALS certification required within 60 days of hire. 2 years of nursing experience required in ICU/Emergency Room/Recovery Room or other critical care-nursing environment Is aware of professional nursing standard and performs in accordance with them. Knowledge of commonly used concepts, practices, and procedures within a particular field. Knowledge of state, federal and accrediting body regulations and standards related to healthcare. Ability to relate and work effectively with others. Ability to read, write and communicate effectively in English, with additional languages preferred. Must be computer literate with basic fundamental computer skills that are required to perform essential job related duties. Duties and Responsibilities: PEOPLE Adheres to staffing schedules, attendance policy, and cooperates with daily assignments as requested. All full-time Registered Nurses are required to participate in the On-Call rotation for PACU. Demonstrates skills as a team player. As a Registered Nurse, shows appropriate delegation, while directing and supervising ancillary personnel in planning, implementing, monitoring and evaluating patient care in the pre-operative/post-operative clinical areas according to accepted standards. May be requested to perform charge duties. Provides direct patient care in the assigned clinical areas and demonstrates clinical competence. Participates in orientation of new staff, ensures facility morale exemplifies the mission and values of the center. Demonstrates ability to solve problems timely and constructively; maintains focus on improving the situation. Willingness to teach others and participate in competency programs. Completes Cornerstone modules in a timely manner annually. Participates in all staff meetings for department and facility for informative and educational purposes. Maintains skills required to be fully cross-trained in all areas of PeriAnesthesia and participate in the week-end PACU call schedule unless employment status is PRN. SERVICE Demonstrates highly effective communication skills with staff, management, physicians, patients, and family members. Establishes and maintains exceptional services to all customers including physicians, patients and employees with the goal of delighting all customers. Participates in service recovery by taking ownership of the situation and correcting problems. Keeps customers informed of progress related to clinical operations and provides comfort, information, and assistance during the process. Models appropriate interaction with physicians, patients and staff to ensure high level of service in all areas of responsibility. Reviews daily schedule and prepares patient care areas based upon types of procedures and specific patient care needs. Communicates changes in status as necessary to physicians, family members, and other staff. QUALITY Initiates physical and psycho/social assessment and pre-operative plan of care, identifies patient and obtains informed consent. Completes and documents a pre-operative interview, obtains necessary diagnostic test results and notifies appropriate physicians and staff regarding variances and/or concerns. Completes and documents post-operative phone calls and reports problems and/or concerns. Completes the Post-Anesthesia record and assures all patients have met the discharge criteria prior to discharge; and provides all patients post-operative instructions. Assures the delivery of quality clinical care through participation in SP benchmarking programs. Assures facility clinical policies and procedures are followed. Maintains a safe clinical environment, adheres to all patient care safety requirements. Reviews and participates in performance improvement initiatives. Medical record documentation is accurate, legible, and in accordance with federal and accrediting standards. Participates in quality improvement activities. FINANCE Conserves resources and minimizes cost when possible. Participates and assists with implementation of cost saving initiatives for the department. Communicates ideas for cost containment and reducing expense. Appropriate staffing levels and overtime is below established benchmarks. Medical record documentation should be completed accurately for all cases. GROWTH Through competency and customer service, employee will contribute to the growth of the facility. Employee communicates service line development ideas with facility leadership. Employee engages in customer service conversations with physicians as adjunct to facility marketing initiatives.
