Graduated from an approved school of professional nursing

Required Certifications and Licensures:

Registered nurse with current licensure to practice nursing in the practicing state.
Previous experience in home care setting with two years management or supervisory experience, preferred.
Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements

Overview Description : Under the supervision of the Ambulatory Administrator or Director, Manager and Medical Director, utilizes the Nursing Process to provide care, coordination of care and patient education to a group of patients, using the medical home mode.
