Responsibilities:

  • Accompany medical practitioners during patient visits and record detailed notes
  • Document patient history, symptoms, physical exams, and procedures
  • Enter diagnoses, treatment plans, and follow-up instructions into the EHR
  • Review charts for accuracy and completeness
  • Retrieve lab results, imaging reports, and prior records for the provider
  • Assist with administrative tasks such as discharge instructions or referrals
  • Maintain patient confidentiality in compliance with HIPAA regulations
