Case Manager (Skilled Nursing) – Managed Care-HMO (RN/LPN/LMSW/LCSW) Qualifications: Active licensure required as a RN, LPN, LMSW, or LCSW Location: Valley Stream, NY (onsite) Compensation: $75,000-$115,000 DOE & Credentialing About the Role We are seeking an experienced and detail-oriented HMO Case Manager to support skilled nursing facility (SNF) patients. The ideal candidate will have a strong background in healthcare, long-term care, or insurance, with at least 3 years of relevant experience.
