What You’ll Do (Impact-Focused Role)

As an LPN, you are a clinical leader and advocate for our residents, supporting both Assisted Living and Memory Care.

You will:

• Conduct resident assessments and develop individualized care plans

• Manage and administer medications and treatments safely and accurately

• Monitor and evaluate resident conditions and adjust care as needed

• Ensure compliance with state regulations and clinical standards

• Provide delegation and oversight to caregiving staff

• Complete required documentation and charting

• Support residents with dignity, compassion, and respect

• Participate in on-call rotation or ensure appropriate clinical coverage

Key Responsibilities

• Complete on-site assessments at least every 45 days

• Provide clinical oversight and task delegation to team members

• Maintain accurate, timely documentation

• Collaborate with interdisciplinary team members to enhance care outcomes

• Support both Memory Care and Assisted Living populations

What We’re Looking For

✔ Active LPN license in Tennessee
✔ Experience in assisted living, long-term care, or memory care preferred
✔ Strong clinical judgment and organizational skills
