Responsibilities:

Provide one-on-one care to clients in their homes, including assistance with daily living activities, personal care, and household chores
Develop and maintain positive relationships with clients, family members, and caregivers
Stay up-to-date on client care plans and ensure that all services are delivered in accordance with our high standards
Collaborate with other caregivers and healthcare professionals to ensure seamless care transitions
Maintain accurate records and communicate effectively with our office team
