Serves in an expanded role to assess patient needs, plan and provide nursing care and coordinate follow-up care for patients receiving treatment in an outpatient Hematology/Oncology setting. Primarily responsible for care navigation throughout the continuum of care. Key responsibilities include patient education, triage and care coordination. $10,000 sign-on bonus for eligible RN candidates. This role is full-time, 40 hours weekly; 8:00 a.m.- 4:30 p.m. 2 years RN work experience is required; previous Hematology/Oncology experience is strongly preferred. Provides instruction to ensure patient's understanding of medical condition and compliance with treatment instructions. Performs and assists with triaging telephone calls concerning health issues and offers appropriate telephone management advice to patients as liaison to physician recommendations. Communicates lab, radiology and other testing results to patients and via telephone and electronic medical record letter notification.
