LPN Resident Wellness Coordinator Responsibilities:

  • Monitor and assess residents’ health and report changes in condition

  • Administer and oversee medications and treatments

  • Develop, update, and manage individualized care/service plans

  • Coordinate medical services, appointments, and outside providers

  • Communicate with residents, families, and healthcare professionals

  • Supervise and train caregivers and medication staff

  • Ensure accurate documentation and regulatory compliance

  • Monitor infection control, safety, and incident reporting

  • Support admissions, transitions of care, and discharges

  • Advocate for resident wellness, safety, and quality of life

     

LPN Resident Wellness Coordinator Qualifications:

•    Must have New York LPN license
•    Assisted Living / Healthcare experience
•    Strong oral and written communication skills
•    Strong computer skills
