LPN/RN Core Responsibilities:

Consults and coordinates with healthcare team members to assess, plan, implement, and evaluate patient care plans
Prepares and administers (orally, subcutaneously, through an IV) and records prescribed medications. Reports adverse reactions to medications or treatments in accordance with the policy regarding the administration of medications by a licensed registered nurse.
Records patients medical information and vital signs.
Monitors and adjusts specialized equipment used on patients, and interprets and records electronic displays.
Initiates corrective action whenever the patient displays adverse symptomatology
Provides bedside care for a wide variety of medical patients.
Changes dressings, inserts catheters, and starts IVs
Prepares equipment and aids the physician during the examination and treatment of the patient
Responds to life-saving situations based upon nursing standards and protocol
Participates in discharge planning.
Utilizes time management skills.
Records all care information concisely, accurately and completely, in a timely manner, in the appropriate format, and on the appropriate forms.
Performs other position-related duties as assigned, depending on assignment setting

LPN/RN Requirements:

Current LPN/RN license in State.
At least one year of LPN or RN experience in a Long-term Care Setting is preferred.
Knowledge of MatrixCare preferred.

Complete compliant documentation of the care provided in the system of record. Manage and administer prescribed medication, treatment, and therapies. Conduct patient assessments, coordination of care Perform various activities associated with daily living. Monitor vitals and GI intake and output. Assess skin integrity and administer wound care. Monitor for changes in the patient’s condition such as weight loss/gain, self-care abilities, and indicators of disease progression. Communicate with family members, physicians, case managers, and other prescribed individuals regarding the health of the patients. Provide education and training to the patient, their family, and/or the Home Health Aide. Work with patients to be done one-on-one in the patients home to provide for a comfortable environment.
