Clinical Leadership & Responsibilities
As a core member of our interdisciplinary team, you will drive the clinical success of your unit:
Clinical Oversight: Monitor vital signs and overall physical/emotional status, identifying subtle changes in condition to ensure proactive intervention.
Advanced Medication Management: Lead the accurate preparation and administration of medications (including injections and specialized treatments) while managing MAR/TAR documentation.
EHR & Documentation Excellence: Maintain high-fidelity electronic health records, ensuring all narratives are descriptive, timely, and compliant with state regulations.
Physician Liaison: Manage physician orders with precision, coordinating seamlessly with the pharmacy to ensure treatment continuity.
Direct Care & Wound Management: Execute skilled nursing tasks including complex wound care, dressing changes, and lab sample collection.
Resident Advocacy: Assist residents with activities of daily living (ADLs) while providing the emotional and psychological support that defines "Complete Care."
Family Communication: Act as a primary point of contact for families, offering clinical updates, comfort, and discharge education.
Qualifications
Licensure: Must hold an active, unrestricted Wisconsin LPN License.
Clinical Competence: Proven experience in a long-term care or sub-acute setting preferred.
Soft Skills: A calm-under-pressure demeanor with a genuine heart for the geriatric population.
Accountability: Ability to make sound independent decisions while working harmoniously with the RN Supervisor and DON.
