Nurse Practitioner (NP)

As a Weekend In-House Nurse Practitioner, you will be supporting the primary provider of internal medicine services within a Skilled Nursing Facility (SNF). You will have the opportunity to build strong, lasting relationships with residents, their families, facility staff, and providers, while being an integral part of the clinical leadership team.

Job Requirements:

Active Illinois Nurse Practitioner License (unencumbered).
Master's Degree in Nursing from an accredited program.
Strong clinical background in primary care and preventive medicine.
Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings.
Ability to engage and communicate effectively with elderly and chronically ill patients and their families.
Ability to maximize onsite time to develop robust caseloads
Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.

Key Responsibilities:
As the Weekend In-House Nurse Practitioner, you will provide essential care and leadership within the facility, including:

Ensuring continuity of care by communicating with the primary In-House Nurse Practitioner
Conducting new admission evaluations and monthly stability visits for long-term residents
Performing comprehensive physical examinations and evaluating changes in patient conditions
Prescribing medications and treatments as needed
Developing patient care policies and facilitating referrals to specialists
Coordinating advanced care planning and discharge visits
Ensuring timely communication and documentation of patient assessments, treatment plans, and follow-ups
Supporting the physician-patient relationship and addressing gaps in care

Our NPs work closely with the interdisciplinary team, taking a leadership role in patient care, collaborating with physicians, and ensuring high-quality service delivery in our facilities.

How you make a difference The Nurse Practitioner (NP) under the supervision of the Site Medical Director and other site Physicians, offers a comprehensive range of medical services to incarcerated patients in accordance with established policy and state guidelines. They ensure necessary documentation of services is provided to the Site Medical Director or designee to monitor the provision of clinical services. Additional Details We are looking for a part time NP to join our clinical team at Will County Jail. Our ideal candidate has experience with MAT/MAR (medication assisted treatment/medication assisted recovery), acute and chronic care experience. This role will provide leadership to the MAR team. Qualifications & Requirements Education A graduate of an accredited school of nursing An advanced degree in an approved and accredited Nurse Practitioner program. Experience Prior medical/surgical and/or emergency/trauma experience and corrections experience is preferred. Must have the patience and tact to deal with the inmate patient population and ability to work effectively in the corrections environment. Scheduling flexibility is also required to be able to rotate hours and shifts, if needed, and to be called during emergency situations to provide coverage. Licenses/Certifications Must have and maintain current licensure as a Nurse Practitioner within the state of employment. Must have and maintain current CPR certification.
