Responsibilities:

  • Provide direct patient care to residents in a rehabilitation or long-term care setting

  • Assess, plan, implement, and evaluate individualized care plans to meet the unique needs of each resident

  • Collaborate with other healthcare professionals to ensure comprehensive care and communication

  • Monitor and report changes in resident condition to the healthcare team

  • Maintain accurate and up-to-date records of patient care and progress

  • Participate in quality improvement initiatives to enhance patient satisfaction and outcomes
