Townhouse Center for Rehabilitation & Nursing is seeking a compassionate and dependable Home Health Aide (HHA) to join our team. In this role, you will provide essential support to residents by assisting with transportation to medical appointments and ensuring their safety, comfort, and well-being during outings.

Job Responsibilities

  • Escort residents to and from medical appointments, including clinic visits and dialysis treatments.

  • Ensure residents’ safety during outings and monitor for potential falls or altercations.

  • Assist residents in preparing for appointments and provide snacks during out-on-pass periods as needed.

  • Report all incidents or accidents immediately to the Clinical Coordinator or Assistant Director of Nursing (ADNS).

  • Represent the facility in a professional manner while following all policies and procedures both on and off-site.

Qualifications

  • Current New York State Home Health Aide (HHA) Certification required.

  • Strong communication and interpersonal skills.

  • Reliable, flexible, and able to work well with residents and staff.

  • Compassionate attitude with a commitment to resident care and safety.
