Summary
We are seeking a dedicated Registered Nurse to join our team in providing exceptional care to our clients. The Registered Nurse (RN) is responsible for supervising non-medical care services provided to clients in their homes. This includes conducting required supervisory visits, updating individualized care plans, overseeing caregivers, ensuring compliance with care standards, and coordinating communication between clients, families, and healthcare professionals. The RN plays a key role in the success of Right at Home’s mission to improve the quality of life for those we serve.
Responsibilities
Provide high-quality nursing care to clients in their homes
Conduct supervisory visits for each client every 30–90 days based on regulations and client needs.
Conduct patient assessments and develop individualized care plans
Collaborate with healthcare professionals and family members for cohesive support
Observe and report changes in clients’ physical or emotional condition to family members or appropriate medical personnel.
Monitor and evaluate Certified Nursing Assistants (CNAs) and Personal Care Aides (PCAs) in the field for performance, adherence to care plans, and quality of care.
Maintain accurate, timely, and complete clinical documentation in accordance with company policies and state regulations.
Build and maintain relationships with clients and families, promoting trust and client satisfaction.
Educate clients and families on health management and wellness
Ensure compliance with health regulations and standards
Participate in ongoing training and professional development.
Oversee the client wellness program, including tracking health metrics and identifying risks or needed interventions.
Coordinate care needs with internal office staff to ensure proper scheduling and delivery of services.
Support and ensure compliance with all applicable local, state, and federal regulations.
Participate in staff meetings, training, and quality improvement activities as required.
Qualifications
Valid Registered Nurse (RN) license
Proven experience in personal or home health nursing
Ability to travel to multiple client homes across Chester, Chesterfield, the Tri-Cities, and Richmond.
Excellent communication skills for interacting with clients, families, and healthcare teams
Ability to work independently and manage time effectively
Compassionate nature with a commitment to client-centered care.
May require standing, walking, bending, and lifting during client assessments or caregiver supervision.
Occasional evening or weekend visits may be required based on client needs or emergencies.
Work Environment:
Field-based role with regular travel to client residences.
Collaboration with office staff in a professional office setting as needed.
