Compensation: Guaranteed Base, Uncapped Potential, $70,000 - $85,000+ commensurate with experience, processed bi-weekly.

Shift Type: Full-time, Monday - Friday, 8 AM to 4 PM (flexible), on-call rotations.

Summary:

Apollo Medical Home Health - a network agency of LiveWell Partners, is a leading independent home care provider seeking a compassionate and skilled Float Registered Nurse (RN) to join our team. As a Float RN, you will have the opportunity to make a meaningful difference in the lives of our patients and their families, while enjoying the flexibility and autonomy of a float position.

Responsibilities:

• Provide high-quality, patient-centered care to patients in their homes, ensuring exceptional clinical outcomes and patient experience scores.

• Collaborates with the patient, family/caregiver(s), ordering physicians, case manager, other care team members, and community resources.

• Participate in case conferences with other clinicians by reviewing services, clinical documentation, and discussing pertinent patient care issues and goals.

• Conduct thorough patient assessments, perform skilled care, and educate patients and families on disease management and self-care.

• Maintain accurate, timely, and detailed records of patient care in the electronic medical record (EMR) system, including progress notes.

• Participate in on-call rotations.

• Develop and maintain positive relationships with patients, families, and caregivers, while promoting a culture of respect, empathy, and compassion.

Qualifications:

• Minimum of 2 years of nursing experience with a preference for 2+ years in an acute or a post-acute care setting.

• Active State Registered Nurse licensure is required.

• Proof of current CPR certification and valid Driver’s license.
