Job Summary:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Axiom Gardens of Mount Vernon. As an LPN, you will be responsible for providing direct nursing care to our residents and supervising the day-to-day nursing activities performed by nursing assistants. This is a full-time and part-time position available on morning and night shifts.

Essential Duties and Responsibilities:

  • Direct the day-to-day functions of the nursing assistants and make written and oral reports as required

  • Admit, transfer, and discharge residents as necessary

  • Provide leadership to nursing personnel assigned to your unit/shift

  • Complete and file required recordkeeping forms/charts and chart nurse's notes in a descriptive manner

  • Prepare and administer medications as ordered by physicians

  • Develop and maintain a positive working rapport with inter-departmental personnel and other departments within the facility

  • Create and maintain a calm and respectful environment throughout the unit and shift

  • Monitor personnel to ensure compliance with safety regulations and maintain confidentiality of resident care information

Qualifications:

  • Licensed Practical Nurse with current unencumbered state licensure

  • Ability to read, write, speak, and understand the English language

  • Ability to make independent decisions when circumstances warrant such action

  • Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities

  • Current CPR Certification is required

  • Previous experience in Long Term Care is preferred
