Compensation: Pay per visit, $27 - $28 per visit, Mileage reimbursed at $0.67/mile Payroll processed bi-weekly (26 pay periods).

Summary:

We are looking for a compassionate Home Health Aide to provide assistance in daily living and personal care services in the patient's home environment in accordance with an established care plan.

 

Responsibilities:

  • Provide health care services in patient residences

  • Perform domestic and household tasks

  • Assist with patient's personal care activities

  • Monitor patients (vital signs, temperature, respiration, etc) and report on their condition

  • Maintain patient care records and document provided services

  • Assist patients with mobility and physical therapies/exercises

  • Instruct and counsel patients and families on diet and exercise

  • Collect routine specimens

  • Provide companionship and basic emotional or psychological support

     

Qualifications:

  • Active Certified Nurse Assistant (CNA) license in the state of employment

  • Current CPR Certification

  • Minimum of one (1) year of professional experience

  • Must have a valid driver's license & proof of insurance

Assume the authority, responsibility & accountability of Activity Assistant/Aide. Ensure that all charted activity progress notes are informative and descriptive of the services provided and indicate the resident’s response to the service. Develop & maintain a good working rapport with other departments within the facility to assure that daily activities can be performed without interruption. Create & maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the facility. Assist in preparing activity rooms/areas for scheduled activity programs. Recommend to your supervisor the equipment and supply needs of the activity department.
