Responsibilities:

Provide personalized care to residents, focusing on their physical, emotional, and social needs
Assist with daily living activities, including bathing, dressing, grooming, and toileting
Monitor vital signs and report any changes to the nursing staff
Develop and maintain positive relationships with residents, families, and colleagues
Implement individualized care plans to meet the unique needs of each resident
Collaborate with other healthcare team members to ensure seamless care delivery
Maintain accurate and detailed records of resident care and activities
