Position Overview

The Hospice RN is responsible for coordinating and delivering skilled nursing care, ensuring comfort, dignity, and quality of life for patients receiving hospice services.

Key Responsibilities

Perform comprehensive patient assessments and develop care plans
Provide direct nursing care and symptom management
Administer medications and treatments as ordered
Monitor patient condition and communicate changes to the care team
Educate patients and families on care and disease progression
Collaborate with the interdisciplinary team
Maintain accurate and timely documentation

Qualifications

Active Registered Nurse (RN) license in Pennsylvania (required)
Hospice or home health experience preferred
Strong clinical and assessment skills
