Job Summary
Right at Home Cleveland West is seeking a dependable and compassionate PRN Licensed Practical Nurse (LPN) to support our Medicaid home care business. This role is ideal for a nurse who values flexibility and enjoys working independently while helping ensure quality care for clients in their homes.

The PRN LPN will play a key role in the management of client care by opening cases, completing supervisory visits, and closing cases in accordance with Medicaid requirements, agency standards, and applicable regulations.

Responsibilities

  • Open new client cases and complete required nursing documentation
  • Conduct supervisory visits to monitor client care and caregiver performance
  • Complete case closings and all associated discharge documentation
  • Ensure compliance with Medicaid guidelines, agency policies, and state regulations
  • Communicate effectively with office staff, caregivers, clients, and families
  • Identify changes in client condition and report findings appropriately
  • Maintain accurate, timely, and thorough clinical records

Qualifications

  • Current LPN license in the State of Ohio
  • Minimum of 1 year of nursing experience preferred
  • Home care experience preferred
  • Knowledge of Medicaid home care documentation and compliance preferred
  • Strong organizational and communication skills
  • Ability to work independently and manage visits efficiently
  • Reliable transportation and valid driver’s license
