Memory Care Wellness Manager (LPN)

We are committed to providing exceptional care and a vibrant lifestyle for our residents. We are seeking a compassionate and experienced Memory Care Wellness Manager (LPN) to lead our clinical team and support the overall well-being of our residents.

The Wellness Manager (LPN) is responsible for overseeing the delivery of resident care, ensuring compliance with all state regulations, and promoting a culture of quality, safety, and service excellence. This role works closely with the Executive Director and other department leaders to support both clinical and operational success.

Key Responsibilities

Oversee and coordinate all resident care services in Assisted Living and/or Memory Care
Conduct resident assessments, develop care plans, and ensure accurate documentation
Supervise and support caregiving and medication staff, including training and scheduling
Monitor resident health status and communicate changes with families and providers
Ensure compliance with state regulations and community policies
Manage medication systems and oversee safe medication administration
Participate in move-in assessments and ongoing service plan reviews
Foster a positive, team-oriented work environment

Qualifications

Current, active Licensed Practical Nurse (LPN) license in Arizona
Minimum of 1–2 years of clinical experience (senior living preferred)
Previous leadership or supervisory experience strongly preferred
Knowledge of state regulations for Assisted Living/Memory Care
Strong communication, organization, and problem-solving skills
Compassionate, resident-centered approach to care
