Position Summary: 

The Concierge Care Navigator is a dedicated liaison for St. Luke’s concierge patients and other patient populations of the organization. This role ensures a seamless, personalized healthcare experience by triaging care needs and coordinating provider visits and other care access points. The ideal candidate is a Registered Nurse with exceptional interpersonal, customer service, and communication skills. Responsible for promoting teamwork with all members of the healthcare team. Performs duties in a manner consistent with St. Luke’s mission and values.  

 

Education, Experience, & Licensing Requirements:   

Education: Bachelor's in Science Nursing or other healthcare related field. 

Experience: Minimum of 3 years of clinical experience, preferably in ambulatory or care coordination settings.  

Licensure: Current licensure as a Registered Nurse in the state of Missouri. Certification in Case Management (CCM) or Patient Navigation preferred.
