Job Title: Licensed Practical Nurse (LPN)
Location: Neptune, NJ
Shift: Full Time, Part Time, Per diem (Weekends)
Position Summary
The LPN is responsible for delivering nursing care within their scope of practice, supporting care plan implementation, administering medications, and supervising nursing assistants. This role requires strong clinical skills, attention to detail, and a commitment to resident-centered care.
Key Responsibilities

Provide nursing care in accordance with facility policies and established standards

Assist in developing and updating care plans under the supervision of an RN
-
Administer medications and treatments as prescribed
-
Monitor residents for changes in condition and report to RN/physician as needed

Accurately record vital signs and maintain detailed documentation
-
Supervise and support Certified Nursing Assistants (CNAs)
-
Participate in daily rounds and collaborate with the care team
-
Assist with resident comfort and daily care needs
-
Observe for and report adverse reactions to medications
-
Perform treatments such as dressing changes, specimen collection, and other procedures within scope

Ensure compliance with infection control practices and follow universal precautions

Respond promptly to call lights and resident needs

Maintain a clean, safe, and organized work environment

Attend required trainings and staff meetings
Qualifications
-
Graduate of an accredited School of Nursing
-
Active Licensed Practical Nurse (LPN) license in the state of New Jersey
-
Current CPR/BLS certification (e.g., American Heart Association)

Previous experience in a skilled nursing facility (SNF) preferred

Strong communication and interpersonal skills

Ability to multitask and work in a fast-paced environment
