Nurse Practitioner or Physician Assistant PRN

As a Nurse Practitioner, you will initiate, develop and implement nursing care plans in accordance with state laws and concurrent with Concentra medical protocols in accordance with Concentra policies, practices and procedures and applicable.

 

PRN flexible scheduling:  You will have the opportunity to contribute to our local Tampa centers during the following shift times, Monday - Friday:

  • Tampa University:  8a-5p
  • Tampa East:  7a-4p; 8a-5p, 9a-6p, 10a-7p
  • Tampa Airport: 7a-4p, 10a-7p
  • Additional locations:  Brandon, Lakeland; etc - visit www.concentra.com/locations

PRN colleagues are permanent team members, and help ensure that our patients always receive consistent care while Full Time team members are CME, PTO, LOA, FMLA etc.  Additional shift opportunity may be available during times that we anticipate increased patient visits.  You may schedule 1-2 months in advance. 

Responsibilities

 

  • Administers prescribed medications and treatments in accordance with approved nursing techniques and protocols.
  • Observes and evaluates patient, records condition and reaction to drugs, treatments, and significant incidents
  • Evaluates outcome of patient care
  • Performs physical examinations and preventive health measures within prescribed guidelines and instructions of physician.
  • May direct nursing care through nursing staff
  • Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy or related therapeutic procedures.
  • Arranges referrals, consultations, therapeutic services and confers with other specialists on course of care and treatment.
  • Draws blood and prepares appropriate paperwork for laboratory pick-up.
  • Performs urine drug screens, using proper chain of custody; ensures preparation for pick-up and shipment to appropriate laboratory.
  • Assists in medical record maintenance by keeping health, administrative, and program records onsite, safeguarding confidentiality of employee health information.
  • Dispenses medications as directed by practitioner and in accordance with state regulations.
  • Maintains and operates all clinical equipment, including calibration and record keeping in accordance with current policy and procedures.
  • Performs ancillary testing and tasks as ordered by the Center Medical Director (PFT, EKG, injections, audiograms, blood draws) in accordance with established policies and procedures
  • Provides excellent customer service to all patients, clients and peers while incorporating Orange Book values.
  • Dedicated to exceeding customer and patient expectations as measured by NPER/NPS/TATs.
  • Ensures accuracy when completing and executing tasks such as drug screens, DOTs, bandaging, dressings and performing quality assurance checks.
  • Ensures accurate, concise, timely and complete documentation of results and paperwork.
  • Provides an excellent, compassionate and warm patient experience regardless of patient volume.
  • Ability to manage time, prioritize and multi-task in a busy environment
  • Ability to adapt to changing business needs such as scheduling and working additional/different hours/shifts when appropriate.
  • Ability to consistently deliver quality care in a busy clinical environment.

Qualifications

  • Master’s Degree in nursing or related health field from an accredited college or university
  • Graduate of an accredited Nurse Practitioner or PA-C program
  • Current license in the state where employed and in accordance with state laws of practice
  • Board Certification
  • Must be able to treat a patient demographic ages 2 and older
  • FMCSA NRCME requried prior to start date
  • DEA required prior to start dte

Job-Related Experience

  • Customarily has at least two years of demonstrated experience in occupational medicine, urgent care or an emergency setting
  • Proficient in assessment and treatment of minor injuries, including minor procedures such as suturing, i&d, foreign body removal, etc
  • Skilled in evaluation of musculoskeletal injuries and interpreting xray

As an advanced practice provider (APP) within integrated home care, you will help enhance the well-being of frail, medically complex, and often homebound patients. You will conduct comprehensive in-home assessments, develop individualized treatment plans in close collaboration with the patient's care team, and leverage both in-person and telemedicine capabilities to deliver care. Your expertise will directly help enhance patient outcomes, reducing acute care utilization, and promoting a higher quality of life for our vulnerable patient population. {You will cover the hub of Plant City, Zephyrhills, Winter Haven, and Lakeland.} Responsibilities: You will conduct comprehensive in-home evaluations and examinations by performing assessments of enrolled patients, focusing on their medical, functional, and psychosocial needs. You will develop and implement personalized treatment plans by collaborating with primary care physicians and other members of the interdisciplinary team to create treatment strategies tailored to each patient's unique circumstances. Co-management with the patient's PCP. You will educate and empower patients and families by providing education about their conditions, treatment options, medications, and the benefits of integrated home care, empowering them to participate in their care. Manage acute clinical issues in the home: Diagnose and treat urgent conditions, such as pneumonia, CHF/COPD exacerbations, UTIs. You will administer and monitor medication administration in the home setting, following established protocols and ensuring patient safety. You will maintain accurate documentation by documenting all home visits, assessments, interventions, and communications within our Electronic Medical Record (EMR) system, Athena You will participate in medical team meetings by contributing to collaborative discussions, sharing insights, and participating in care planning during scheduled team meetings on a rotating basis. Based on coverage area, you will have a caseload of 45-50 patients in a given month, 200 unique patients per year, completing six-eight visits per day, establishing a weekly or every-other-week cadence of visits with patients in the caseload. You will share after-hours call responsibilities and provide telemedicine consultations to patients and families as part of a rotating on-call schedule. Use your skills to make an impact Required qualifications: Must be a nurse practitioner or physician assistant with minimum of five years of experience in acute care, geriatrics, palliative care, or a related field. You will need to have demonstrated experience working with geriatric patients and their families in the home setting, with a commitment to providing compassionate and patient-centered care. Experience and comfort with advance care planning and end-of-life discussions. You will need experience ordering medications in the home setting. You will need to have prior experience with building relationships and trust with patients, families, and colleagues. You will need to excel in a collaborative, diverse team environment, contributing your expertise and working with diverse professionals. You need to be proficient in using electronic medical record (EMR) systems for accurate and efficient documentation. You must be board certified as a nurse practitioner or a physician assistant with a clear and active Florida license, or obtain the license. Work Schedule: Regular business hours: Monday through Friday, 8:00 AM to 5:00 PM. After-hours call is shared equitably among the direct patient care team. Scheduled Weekly Hours 40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $115,200 - $158,400 per year This job is eligible for a bonus incentive plan.
